Announced

CMS and Labor Department announce five large employers' support for electronic prior authorization in self-funded plans

Announced Oct 21, 2026; related payer API requirement takes effect Jan 1, 2027

From Briefing for the week of October 20 – October 26, 2026

What changed

CMS announced that Amazon, Axon Enterprise, Google, Meta and Oracle signed a statement of support for making the electronic prior authorization capabilities developed under the 2025 insurer pledge available to employers that sponsor self-funded health plans. The release sets no deadlines, lists no specific required actions and describes no enforcement mechanism. Healthcare Dive reports that nearly 60 health plans signed the 2025 voluntary pledge and say they have cut prior authorizations by 11%, a figure the plans themselves reported, and that some providers say they have seen no improvement. The release separately refers to a rule requiring certain payers to implement electronic prior authorization beginning Jan. 1, 2027.

Why it matters

Self-funded employer plans, governed by ERISA, were outside many earlier prior authorization reforms, so employer commitments could extend the reach of the effort, but this is a voluntary statement, not a requirement. The effect on denials, delays or patient outcomes is not measured in the announcement, and the 11% reduction is self-reported by insurers.
